STORAGE, ACCESS AND TRANSFER OF CLIENT RECORDS
A client or insurance company may request client records by notifying me in writing by mail (USPS) of such request. Faxes or email for such request will not be honored. Please use the legal address as listed above. There is a $100.00 copy, mailing and administrative handling fee is associated with this request which is payable at the time the request is made (in advance). Your request should include the name, address of the professional to whom these records are to be sent and, the reason for making such request, along with $100.00 in check or money order. You may be asked to complete and sign an Authorization to Release Confidential Information Form, which will be either emailed or faxed upon request.
I reserve the right to refuse to release these records if it may result in a danger to self or others because of the sensitive nature of the information in these records. I reserve the right to recommend that we meet to discuss what I i the records and to help you understand the information. Consultation for this purpose is billable to the client at the regular rates and is not covered by insurance.
Please send to CCS Operations, P.O. Box 14524, Mesa, AZ 85216. Due to postal service inconsistencies, I cannot guarantee requests sent to the service address will be received. If payment is included, I will respond within one week of receipt of your request. Records will not be released until payment in advance is made.
Record of Services rendered under Employee Assistance Program (EAP) are NEVER released by CentrePoint/Marco Cuyar, MS LPC LISAC, as per my provider contract with the EAP providing companies. There are no exceptions to this policy. I will be happy to provide you with a referral if this is a service you are looking for. I are unable to complete disability forms or leave of absence paperwork while using EAP services.
Records are kept in a locked filing cabinet (in a locked office and suite with an alarm which is set on a daily basis) for up to one (1) year of last date of contact and/or service, and at another secured storage location from my office after one (1) years of date of last contact and/or service. Records for adults are kept 6 years after date of last contact/service. Records for minors are kept for 3 years after he/she reaches 18, or at least 6 years after the date of last service and/or contact with said minor, whichever is greater, pursuant to ARS 12-2297. Clients will be informed within 60 days of the termination and/or sale of my practice in writing via U.S. Postal Service to the last known mailing address you have provided to this therapist. Please call (480) 497-4720 for information about how to obtain your records after termination and/or sale of my practice. I reserve the right to deny access to medical records at any time pursuant to ARS 12.2293. Please check back to this website for latest information.
