INSURANCE AND PAYMENT
CentrePoint accepts MajorPreferred Medical (e.g., BCBS, Aetna, Cigna, United Behavioral Health (UBH), and Beacon Health), Managed Care insurance and Employee Assistance Programs (EAP). Check with your insurance company to make sure I am an in-network provider before scheduling an appointment. If I am not, you may have out-of-network benefits which will still allow me to provide services.
AZ Medicaid (AHCCCS) behavioral health services are only handled through State of Arizona. You should contact your assigned AHCCCS case manager for a listing of approved agencies and a referral.
Counselors are currently not able to provide behavioral health services for Medicare participants.
CentrePoint provides billing services for no additional charge as a courtesy however, you are responsible for knowing what your benefits are and for paying for all services that are not covered by insurance. You are ultimately responsible for all co-pays, co-insurance and any unpaid charges.
PAYMENT OF SELF-PAYS, CO-PAYMENTS, CO-INSURANCE AND DEDUCTIBLES IS DUE AT THE TIME SERVICES ARE RENDERED
IMPORTANT NOTICE: Beginning January 1, 2017, CentrePoint Counseling Service, PC will require either a credit card or cash deposit on file. This allows us to avoid having to take legal or collection actions against any client. No charge will be billed to this account unless the client fails to recocile debts to CentrePoint Counseling Services, PC.
CentrePoint accepts cash, check, credit, debit and Health Savings Accounts (HSAs). There is a minimal convenience processing service charge for the use of debit and credit cards.
